Cost is $25 | CASH, OR CHECKS MADE TO LOVELADY ISD WILL BE ACCEPTED

PLEASE HAVE YOUR MEDICAL HISTORY SIDE OF THE UIL PHYSICAL FORM FILLED OUT UPON ARRIVAL. Forms may be picked up in the HS Office, from Coach Bird or Coach Johnson, or printed from https://www.uiltexas.org/athletics/page/pre-participation-physical-evaluation

All Incoming 7th, 9th, and 11th Grade students or those who have not had a physical are required to have a new physical.

Any 8th, 10th, or 12th Graders that wish to get one may also participate.
